Compounded vials: refrigerate throughout
Compounded GLP-1 is usually dispensed as a multi-dose vial you draw from with a syringe. Every reputable source says the same thing: fridge before opening, fridge after opening, and do not freeze. The peptide degrades with heat, and a vial that has been warm may simply contain less active drug than the label says, which shows up as weaker effect rather than any obvious change in the liquid.
The shelf life after opening is the part that varies. A compounding pharmacy assigns a beyond-use date to each batch, and that date depends on the formulation and the pharmacy's own stability data. Thirty to ninety days refrigerated is the usual range; your pharmacy's figure is the one that counts.1 If your provider did not tell you the beyond-use date, ask — it is a reasonable proxy for how carefully the pharmacy is run.
Brand-name pens: a room-temperature window after first use

How to tell if it has gone off
Often you cannot. Compounded semaglutide is normally a clear, colourless liquid. Cloudiness, particles, or a colour change mean discard it. But heat-degraded peptide can look identical to good peptide, which is why the storage rule matters more than inspection. The practical signal is a loss of effect: if appetite suppression fades on a vial that spent time warm, that is the likely reason.
