Why coverage is so difficult

Many US plans exclude weight-loss medication as a category, regardless of medical need. Where coverage exists it often requires prior authorisation, documented BMI thresholds, evidence of previous attempts at weight loss, or a diabetes diagnosis rather than an obesity one. Appeals are slow and frequently fail.
